Medication and food request form

Medications, food, flea/worm treatment can be requested via this form. Please allow at least 24 hours for your prescription to be filled for routine medications and food. Special order medications can take longer. We will text you when your medication is ready for collection. If your pet has not been seen in the last 6 months, you will need a check up with the vet to ensure we are prescribing the correct medication. For flea/worm treatment we require your pet has been checked by a vet in the past year.
